Lafeber Company was proud to sponsor the Association of Exotic Mammal Veterinarians Student Case Report Contest. Veterinary students from all over the world were encouraged to write a 2-page case report about an exotic companion mammal seen at their college of veterinary medicine or during a clinical experience. Submissions closed March 22 and judges from the Research Committee evaluated the 14 case reports received. Judges were blinded to the students, mentors, co-authors, and institutions at which the cases were seen. See the brief summaries of each winning case report. Each student has been encouraged to submit their paper for publishing and for a presentation at ExoticsCon 2020.
A 3-year old intact male guinea pig was presented on emergency for suspected bloat and with a history of chronic hair loss. Clinical examination revealed non-pruritic symmetric truncal alopecia, thin skin, severe cachexia, and an abdominal fluid wave. An adult intact female guinea pig was presented with dysorexia, exhaustion and weight loss. Radiographic and ultrasound findings demonstrated a severe gas dilatation of the stomach, severe gallbladder distension, and abnormalities on the organ topography. An eleven year old male neutered rex rabbit presented with lethargy and inappetence of two days duration, and an acute episode of vestibular ataxia. Anemia and elevations in plasma alkaline phosphatase and alanine aminotransferase were evident. Abdominal ultrasound revealed questionable, diffuse hepatomegaly. Computed tomography revealed hepatic venous congestion, severe dilation of the pre-hepatic caudal vena cava, and bicavitary effusion, consistent with Budd-Chiari-like syndrome. Turbulent blood flow within the dilated segment of the caudal vena cava was present on spectral Doppler evaluation. Gastric dilatation or “bloat” and gastrointestinal obstruction is an acute and life-threatening condition of pet rabbits commonly caused by an obstruction with pellets of compressed hair. The discussion portion of this Case Challenge reviews onset, clinical signs, and diagnostic test results of obstructive and non-obstructive gastrointestinal disease. This condition is considered a surgical emergency and key points of urgent care strive to stabilize the patient through analgesia, decompression when indicated, and supportive care. Surgery is discussed as well as recommendations for patients that cannot go to surgery due to clinical or financial constraints. Follow-up care as well as homecare recommendations, disease prevention, and prognosis are also explored.

Gastrointestinal problems are common in the pet rabbit. A thorough history, including a detailed dietary history, can provide invaluable clues to the problem at hand. Signs of gastrointestinal discomfort in the rabbit may include bruxism, reluctance to move, and anorexia. If there is a history of anorexia, it is imperative to differentiate whether the rabbit is not eating because it has no interest in food, or if it is showing an interest in food but unable to eat. A complete lack of appetite is most commonly seen with physiological problems such as renal failure, whereas a reluctance to eat is a classic presentation in rabbits with dental disease.
Normal chest radiographs can be challenging to evaluate and easy to over interpret in the rabbit. The thoracic cavity is small relative to the abdomen, and the heart takes up a large portion of the thorax. The cranial border of the heart is less distinct due to the presence of the thymus, which persists throughout the life of the rabbit. The cranial lung lobes are small and are obscured by a wide mediastinum. The caudal lung lobes have a pronounced vasculature. Additionally rapid breathing makes it difficult to obtain an inspiratory film unless the rabbit is anesthetized and intubated.
Heavy metal poisoning in birds most commonly occurs from ingestion of substances containing lead, or less commonly zinc. Acute heavy metal toxicity is occasionally seen in companion parrots that ingest or chew on objects containing metal because of their curious nature and innate desire to forage. Chronic lead poisoning most frequently affects free-ranging wildlife such as ducks, geese, swans and loons and is most commonly seen during migration in the late fall and early spring. Lead toxicity also occasionally occurs in upland game birds such as mourning doves, wild turkey, pheasants and quail. Lead poisoning has also been reported in raptors, presumably from the ingestion of lead-contaminated prey.

This case study is based on a report prepared by Veterinary Answers Consultant, Gwen Flinchum, DVM, Dipl. ABVP (Avian). A 5-year old female umbrella cockatoo is presented for lethargy, open-mouthed breathing, and difficulty gripping of 2-3 days duration.
The pathogenesis of otitis is often multifactorial in the rabbit. Predisposing factors such as ear conformation increase the risk of otitis in certain breeds. All rabbits have a relatively narrow ear canal, however in Lop-eared rabbits the fold in the ear cartilage is such that the lumen is entirely closed off preventing normal drainage of cerumen from the ear.
Dental problems in rabbits and rodents are often related to either trauma or lack of normal wear and tooth elongation. When herbivores like rabbits, guinea pigs, and chinchillas receive concentrates, in the form of grain or pellets, with only limited access to hay and natural vegetation this diet provides too little tooth wear to compensate for the natural growth of the teeth.
After recognizing a dyspneic bird, the clinician’s initial response should be: Hands Off!! Dyspneic birds can die soon after presentation with the additional stress of restraint and handling. Therefore minimize handling and place the bird in an oxygen-rich cage. Humidify air and provide 40 to 50% oxygen. As in mammals, oxygen therapy is potentially toxic if given for prolonged periods at high levels.
